Congratulatory Message Request Form

As a service to constituents of Wentworth, Malcolm will send letters of congratulations to people turning 90 years of age or more and couples celebrating 50 or more years of marriage.

Prime Minister Turnbull's office will also arrange for messages of congratulations from the Queen and Governor-General on a 100th Birthday. On request, the Prime Minister and Governor-General will write again every year and The Queen will write again every five years. The Queen sends messages to couples on their 60th wedding anniversary.

To assist Malcolm’s office, messages should be requested six weeks from the date of the anniversary or birthday. Congratulatory message requests must be accompanied by either a birth/marriage certificate, scan of drivers license or passport or a statutory declaration to the effect that you believe a birthday or anniversary is to take place on a particular day. These can be send by post or email.
